We are located in the Dental Tower of University of Florida Health Science Center in room D7-6. You may contact our administrative office at 352.273.6750.

An appointment for a consultation/evaluation with a faculty member or resident may be made by calling 352.273.6731, option 8. Oral surgery is a referral clinic and should receive referrals by fax at 352.392.6909 to the attention of the Receptionist. Appointment date, time and appropriate forms will be sent via mail.
